MC5
Welcome to the Missouri Coalition Celebrating Care Continuum Change. Our passion is to create a person-directed culture within a health care system, where the dignity and value of each individual who lives and works there is respected and celebrated. A model of care where residents are offered choices and staff is empowered, resulting in improved quality of care.

Nursing Home Quiality Campaign
More than 4,700 nursing homes have already signed on for Phase 2 of the Campaign after just three months of recruitment! More than 3,500 of these nursing homes were rejoining the Campaign, a move that qualifies them for Charter Member status on the Advancing Excellence website. The Campaign is considered to have such an impact on improving organizational culture and clinical care outcomes that the Honorable Jay Nixon signed a proclamation designating the month of January to be Advancing Excellence in America’s Nursing Homes month in Missouri.
To re-enroll for the Campaign, go to www.nhqualitycampaign.org. If you have forgotten your user name and/or password, please contact Jennifer Reed at jreed@primaris.org or 573-817-8300 ext. 182.
As a campaign participant, you will have access to a wealth of free resources, including web-based education sessions, tip sheets, newsletters and updates on the most current research on how to improve patient care, staff satisfaction and public trust. You’ll also be recognized on the website and have access to a packet which includes marketing ideas to promote your participation (press release, decals, buttons, etc…).
With your help, this campaign will improve the quality of life for the more than 40,000 residents in Missouri and the 1.5 million plus residents nationwide! We know that you care about quality and you work to improve care everyday - so take credit by signing up for the AE campaign. To see if you were signed up last year and see a list of who has signed up this year, visit www.nhqualitycampaign.org and click on “Find Participants.”
Creating Home in the Nursing Home II: A National Symposium on Culture Change and the Food and Dining Requirements - February 11th, 2010 is sure to be a date to go down in culture change history, because it is the date for Creating Home in the Nursing Home II - A National Symposium on Culture Change and the Food and Dining Requirements. This will be the single-most important dining and culture change event of the last ten years. Creating Home in the Nursing Home II is a joint effort by the Pioneer Network and CMS to provide education and support to providers, researchers, policy makers and consumers who are involved in culture change initiatives. Innovations including liberalized diets, buffet-style service and reductions in bib and supplement use have been welcomed by providers, but they often struggle integrating new approaches with the regulatory guidelines. This symposium will address culture change dining initiatives and explore potential regulatory barriers so that both innovators and regulators can come to a common understanding of responses congruent with OBRA '87.
